We are a preferred provider for Delta Dental and United Health Care Medicare Advantage. We gladly submit to all insurance companies and we are happy to review your policy with you to assist in determining your benefits. Just bring a copy of your policy and we will explain your benefit options. Insurance does not guarantee payment and some procedures may not be covered. Insurance is an agreement between you and your insurance company; however, we are happy to assist you obtaining benefits for covered services. A 20% co-pay will be collected the day of service for patients with insurance.
Other Payment Options:
We offer a 5% discount for those patient's paying by cash or check the day of treatment. We also accept VISA, Mastercard, American Express and Discover, as well as payment from your HSA. For those patients with no insurance, ask about our North Country Dollars Membership Plan.
As a courtesy to other patients waiting for appointments, we request 48 hour notice for all cancellations or rescheduling. A $25 re-appointment fee may be charged on a case by case basis in the event of a short notice cancellation.
